Eligibility
Eligibility Services
The Eligibility department at CentroMed offers application assistance to identify state programs, insurance or discount programs that patients may be eligible for. Our team specializes in helping you navigate the application process and submit all necessary documentation. To determine your eligibility, we’ll need information about your income, family size, and other relevant details. You can apply in person or online through our website.
If you are having problems paying for your visit, please call 210-334-3745 to schedule an appointment with an eligibility specialist. You may qualify for a sliding fee scale based on your income and family size. Patients will not be denied services based on an inability to pay.
