Prevent T2 Program
If you have prediabetes or other risk factors for type 2 diabetes, it’s time to take charge of your health. The Prevent T2 lifestyle change program can help you make lasting changes to prevent or delay type 2 diabetes.
The Prevent T2 lifestyle change program can help you lose weight, become more physically active, and reduce stress. The Prevent T2 program is a Centers for Disease Control (CDC)-recognized program designed to cut your risk of Type 2 diabetes in half. It uses over 25 years of scientific research on proven strategies for preventing or delaying the onset of Type 2 diabetes.
Risk Factors
- Are 45 years of age or older
- Are overweight
- Have a family history of type 2 diabetes
- Are physically active fewer than 3 times per week
- Ever had diabetes while pregnant (gestational diabetes) or gave birth to a baby that weighed more than 9 pounds

With PreventT2, you get:
- A proven program to prevent or delay type 2 diabetes

- A year-long program with weekly meetings for the first 4 months, bi-weekly for the next 2 months, then – once a month for the last 6 months

Eligibility Requirements
- 18 years or older
- BMI ≥ 25kg/m2
- No previous diagnosis of type 1 or type 2 diabetes
- Diagnosis of prediabetes within the past year based on:
- HbAlC: 5.7% – 6.4%
- Fasting plasma glucose: 100 – 125 mg/dl
- Previous diagnosis of gestational diabetes
- Cannot be pregnant at time of enrollment
